Position Title: Community Health Worker (Promotoras)

Department: Public Policy and Advocacy

Position Reports to: Community Organizer and Outreach Manager

Position Summary: Under the direction of the Community Health Worker (CHW) Manager and Supervisor, the Community Health Worker/Promotor(a) plays a vital role in supporting community members by promoting health, wellness, and access to care through outreach, education, and engagement events. Acting as a trusted liaison between the community and the healthcare system, the CHW fosters strong relationships to encourage healthy behaviors, share critical resources, and improve overall health outcomes. This role emphasizes peer-to-peer interaction, community outreach, and educational efforts aligned with grant objectives. Areas of focus include the intersection of health and immigration policies, community-based advocacy, and expanding access to relevant services.

Responsibilities:

  • Engages the community to support with patient enrollment, build and maintain trusting relationships with members and advocate for their healthcare needs.
  • Proper knowledge of community agencies and resources available.
  • Support community initiatives, projects, and activities that seek to better social and health issues in the community.
  • Acts as a liaison between patients/families and health providers by following organizations values and mission.
  • Provides patient education at external outreach events and assists with the delivery of community resources, including health insurance, housing, food, and quality care information.
  • Provide basic and informal counseling, support, and follow-up with the community.
  • Communicate effectively and professionally when engaging with the public
  • Work collaboratively with internal departments as well as community-based agencies.
  • Ability to organizing resources and establishing priorities.
  • Connect people to health care and social service resources.
  • Assist to build understanding of services available to support healthier decisions and lifestyle choices.
  • Provide health information using clear and relevant terms that are easy to understand.
  • Performs all other duties as assigned including participation in special projects and research.
  • Participate in Clinica activities and events as needed.

Qualifications/Requirements:

  • Ability to work effectively with people.
  • Strong verbal, written, and interpersonal skills; ability to display empathy for other.
  • Written and oral fluency in English and Spanish.
  • High school graduate or equivalent required.
  • Demonstrate commitment to serving underserved population.
  • Ability to work well in a team setting and independently.
  • Strong follow-through and relationship building.
  • Ability to manage events and projects, coordinating multiple priorities, deliverables, and timelines.
  • Proficient in Microsoft Office and familiarized with the use of electronic gadgets like tablets ands smartphones.
  • Experience working in a community-based setting for at least 1 year preferred.
  • Must be adept to respond to evolving policy landscape and its applicability to work and outreach materials.
  • Must be comfortable with peer-to-peer engagement, creative event planning and execution.
  • Self-starter/ comfortable with independent work
  • Comfortable walking/ standing for long periods of time for canvassing and outreach events.
  • Availability/flexibility on weekends.
